The ER is the right call if there is spreading facial swelling toward the eye or down the neck, a fever above 101°F, trouble swallowing, or trouble breathing. For severe tooth pain alone, an ER will only give you antibiotics and pain medication, then refer you back to an endodontist — typically costing $1,500–$2,000 with no actual treatment. If it is safe for you, an over-the-counter NSAID such as ibuprofen or naproxen may temporarily help; follow the package directions and do not combine NSAIDs. Acetaminophen may be an option when NSAIDs are not appropriate. Sleep with your head elevated on two pillows — lying flat raises pulp-chamber pressure and worsens throbbing. Cold compress, not heat.
