How the estimator works

We start with the specialist fee range for the procedure you selected. These ranges reflect what a specialist endodontist actually charges in the South Bay in 2026 for the corresponding ADA procedure codes (D3310, D3320, D3330, D3346–D3348, and D3410–D3428). Specialist fees are higher than general-dentist fees because every operatory is built around a Zeiss surgical operating microscope, in-office CBCT 3D imaging, ultrasonic instrumentation, and bioceramic sealers — equipment and training that drive the 90%+ first-time root canal success rate published in the endodontic literature.

We then apply your carrier's typical out-of-network "allowed amount" — the dollar figure your reimbursement percentage is calculated on. Carriers vary: Guardian and MetLife are usually close to our actual fee, while Humana and some Anthem plans use a thinner schedule. Finally, we multiply by the coverage tier you selected (50% Major or 80% Basic).

Common questions about cost

Why is the range so wide? Specialist fees vary slightly by complexity (number of canals, calcification, anatomy). The reimbursement range reflects real variation across employer-specific plan designs even within the same carrier. We narrow the range to a specific written estimate for your case after a quick phone call.

What if my deductible isn't met? Your first $50–$100 of treatment in the calendar year usually goes toward the deductible before insurance reimbursement begins. The calculator does not subtract this — your real out-of-pocket may be slightly higher early in the year.

What if my annual max is almost used up? Most PPO plans cap reimbursement at $1,500–$3,000 per calendar year. If you've used most of your max, the calculator will overestimate your reimbursement. Tell us at the consultation; we sometimes sequence treatment across plan years.
